Medical Records

All patients have rights when it comes to their medical information. The Health Information Management (HIM) department at UT Health San Antonio maintains the confidentiality of all patient records. They also help you request and receive your medical documents.

Medical records are private health information that cannot be shared with anyone or any organization without the patient's permission or the permission of someone allowed to act for the patient (unless authorized or required by law).

View medical records

The easiest way to see most of your health records is with MyChart. In MyChart, you can view: 

  • After visit summaries and notes 

  • Immunization (vaccine) history 

  • Messages from your provider 

  • Test, lab and imaging results 

  • Other personal health information

Request patient records

To request patient records, you must complete an authorization form. You can find the authorization form in MyChart, or download the form as a PDF. Access the PDF in Spanish.

If you do not have the software to view and print the PDF form, you can download the Adobe plug-in for free. You can also visit the Health Information Management office to complete the form.
